A little bit of a challenge for all you Spanish Linguistic Daredevils out there...
I posted a question, a few days back, asking for help with a translation in reference to this form:

The context; It's a form I was sent by a University in Spain, as I'll be doing a 3 month exchange with them in September.
I only need the sections highlighted in green, to be translated. The rest I've managed to decipher, quite successfully I hope. There are several notes, and a few uncertain translations where some reassurance is craved.
Deface the PDF and link it. Post purely the Spanish translations in order. However, you feel is best
